Has someone borrowed something and still not returned it? If so, what was it?

Yes, my X-Files DVDs. It's been years though and I don't even know where he lives.

Yes.

When we lived in Delaware, I got to the point where I didn't loan anybody anything that I might ever want back.

It just worked better that way.

It is also how I ended up with duplicate tools, because, on odd occasion, somebody would return the stuff they'd borrowed, all at once, and usually after I'd replaced something they had borrowed.

In one case, a friend returned a drill he'd 'borrowed' about three years later. I let him just keep it because I was getting ready to move out of state.

A leather jacket, money and a book. (Not all to the same person)

I lent a leather jacket to a guy where I was working once , said he was going to a 50's party.
Next time I saw it was in the window of the pawn shop.

For people who lent money: How much money was it?

Some people who get things borrowed aren't open-minded so they end up doing silly things like that. That shirt was lent in 2005 and has never been returned since then. Now (but I've been doing it since 2010) my clothes aren't to be borrowed to anyone else.
