Rome doesn't fall.
The Goths and Gauls integrate into Roman society slowly changing it to be more like theirs. The Roman Empire continues to exist as a (successor) city state that controls a swath of central Italy. Like several other city states it eventually transforms into a republic of sorts. When the Italian states are united after Napoleon, they agree to join the continuously free nation of Rome, forming the nation of Italy as a direct successor state of the Roman Empire and Modern Roman Republic.
