Physical, emotional and mental abuse. Failure to provide food, clothing, a safe and loving home, and lack of interest/supervision.

How far is too far before a child should be taken away?

All of the above is too far (IMO). People who notice a child is being neglected and ignore it are just as guilty as the parents. It takes one phone call to child protective services but yet so many people can't be bothered to pick up the phone and help a child.

Neglect?

Well, neglecting things obviously... If a child has no food, or isn't cared for properly. Basically what Jazzy said.
 
You cannot rely on Children's Services to monitor the care of children.

A worker had gone to the home and had observed all children and stated they appeared to be doing well and that there was not a 12-year-old-boy on house arrest. The allegation that had come in was deemed unsubstantiated.
