I am a firm believer in peoples right to die, if they have a very poor quality of life due to serious untreatable condition then they should have the right to checkout when they choose.

This.  As long as they've given it all they've got, and if fighting is no longer an option, then they should be able to pass away on their terms before whatever disease it is takes them.

What if the person is unable to make that decision, say, due to a stroke, brain injury from an accident, or advanced dementia, and they do not have a "medical directive" / "living will" in place.

Then what?
 
If the person does not have a "medical directive" / "living will" in place. then nothing. No one else can step forward and make the decision for them even though it's the right decision. This is why it's very important, at any age, to have your wishes be made legally.

I agree. When ever I have enough of this bull shit world and I decide early on that its my time. Thats my right and not no one is going to debate that with me. Its everyone's right. So if I ever become a vegetable. I give my loved ones full on permission to smother me with a pillow. No way in hell I'm going to live that way.
