Blood and Blood 2: The Chosen comes to mind. Caleb, the main protagonist, is basically a bad guy who has a bone to pick with the other bad guys. I've only played the sequel, and always made sure not to so much as hurt civilians, much less kill them, even though there's no any punishment or anything like that for killing them. I just couldn't bring myself to harm them. Heh.
