Let's compile a list of common kitchen mistakes and mishaps.

Do you have any?

Example topics:
-What types of ingredients/ foods should not be mixed?
-Should I heat water to boiling by itself before putting anything in there (like pasta)?
-Should I preheat a skillet before cooking with it or put food into a cold pan and then fire up the burner?

Preventing cross contamination by properly storing food and washing hands before and after handling different types, not using the same surfaces for things like meats and veggies, etc.

I practice this as well.
Not covering things right and tight before it goes in the fridge is one of my pet peeves.

Agreed; if people would simply do that, there'd be a lot fewer cases of food poisoning and sickness in that regard.
