Most frustrating way to die?

Jumping puzzles that result in your untimely demise if you press the jump key too early, too late, gain too much (or too little) momentum before jumping, desired landing spot moves out of its position, someone (or something) knocks you off course while in mid air, etc.
 
I had a lot of frustration with Guild Wars. For one, a low level cap...that being 20 and having to rely on other players to get through a stage. Too much aggro and you're dead. You basically get swarmed by enemies with level 30 and over.

That's mostly a l2p issue though. Throughout the game there were many decent soloing builds (55hp monk, assassin) and once heroes came into play fights became quite easy in general. Ofc having to rely on other people is logical there, since it's supposed to be multiplayer.
