You only have one seat open in your car, who do you take?

If I can realistically save the person experiencing a medical emergency a ride, then so be it.

Otherwise I would pull over and support both my friend and the love of my life with the medical emergency so as not to leave them on their own.

You got that number? Is what I would say to her and then take the person with the medical emergency to the hospital. Because that's what a phones for and then I would come back and pick her up after. To be honest you'd be surprised how many people I could caravan into a car if I wasn't following the rules of your scenario. We would just pack it in and take both.
