What, for you, is the most frustrating way to die in a video game?

When you're dodging things, like for instance, in the latest Tomb Raider and you have to dodge all the sharp spikes lest you be impaled. Sometimes it's just too damn impossible to avoid, especially if the obstacle is much more complex and required to beat before advancing.
 
Having played some more Skyrim recently, I'm going to answer kill cameras.

When an enemy is low on health attacking them can cause the camera to go into third person while the character plays a special animation to kill the target with one hit. I think they're pretty annoying normally, but they're more annoying when they're used against you:
Once the animation has started you will die even if you fully heal yourself before the blow lands :glare:
 
Glitches/bugs/lag, basically anything that's completely out of my control.
 
Spawn killing, period, full stop.:@
 
Bad servers.

I was constantly dying on Diablo 3 last night because the servers kept going weird, so suddenly a ton of enemies would teleport onto me and crush me. >_>

Ditto. I'm not very fond of these issues either. Oh well, shit happens..
 
I thought of another one. When you're trying to do something like jump to a wall or off a cliff to something else and you accidentally press the wrong button and launch yourself into the abyss. D:

Kinda, it's very lag based. You run out of the cone attack, start casting something and still fly off even if you're like 10 meters away from it. >_>

And there's no reviving you there. =(

Good thing that can't happen to me because my enchanted armor gives me like 500 billion health points. Same goes for my Magicka and Stamina. :)
 
Those parts of games where you're fighting a boss and, unbeknownst to you, it is an impossible fight that you were never meant to win.

Even if you don't enchant your stuff to be ridiculously overpowered it's not much of a late game issue. Steam rolling enemies gets pretty easy :P
 
I had a lot of frustration with Guild Wars. For one, a low level cap...that being 20 and having to rely on other players to get through a stage. Too much aggro and you're dead. You basically get swarmed by enemies with level 30 and over.
